• Identification of the copyrighted work you claim has been infringed
• Identification of the material you claim is infringing, with sufficient detail to locate it

• A statement, under penalty of perjury, that the information in the notification is accurate and that you are the copyright owner or authorized to act on their behalf
• Your physical or electronic signature

4. Counter-Notification

5. Repeat Infringers

Trend Outlet LLC has a policy of terminating, in appropriate circumstances, the accounts of users who are repeat copyright infringers.
